What is average cost basis?

It is the total money spent divided by the total number of shares you hold.

Does this include taxes or commissions?

Only if you include them in the prices you enter. The calculator itself does not add brokerage fees automatically.

Can I leave market price empty?

Yes. The average cost still works without a market price, but unrealized P/L will not be shown.
